Hello and welcome to HTS! I have the same problem sometimes when Wi-Fi is weak or disconnected. Try:
  1. Double-clicking the HOME button (the one on the same face as the screen) VERY fast. Your tablet should display all open apps in their separate windows. Find the blank HTS window/app and swipe it off the iPad's screen to close it.
  2. Open SETTINGS, and make sure you're connected to the Internet through Wi-Fi or Bluetooth
  3. Tap on HTS logo to open app.
  4. Making sure the screen is not zoomed, enter logon credentials.
  5. Viola!

I always use in landscape
Can't login when iPad is in landscape orientation.
In portrait orientation the login fields are visible

I'm sure this can be fixed.

Welcome to Home Theater Shack! Sorry you're having trouble with the App. Sounds like you have found a work around for signing-in. Hope it works well enough!
